Release forms are necessary for all youth ministry events. These forms provide medical information in case of an emergency, as well as release the parish from liability. These forms are in place as a means to keep everyone safe. The necessary form(s) are usually denoted in publicity for events; if you have any question regarding the use of these forms, please contact the Parish Office at (386) 668-8270.
- Photo Release Form
- Should be filled out annually by anyone involved in Youth Ministry (teen, ministry team leader, etc.).
- Medical Consent Form - For all teens 18 and younger, in High School. Must be completed by Parent/Legal Guardian. Please be sure to fill out completely;

Event Release Form - For all adults 18 and older, out of High School. Please be sure to fill out completely.
Driver Authorization Form - For all adults 25 and older. Adults 25 and older are allowed to transport youth; adults 21-24 are not able to transport youth.
