Vacation Bible School 

Registration for Vacation Bible School for grades K-5 in the Summer. Please fill in and submit the form below.  All requirements are for the protection of your child/children and must be met before the student is permitted to attend VBS.  

1. Parental/Guardian Consent Form & Liability Waver (one for each child)
2. Parental/Guardian Medical Information & Concent Form (one for each child)
3. COVID-19 Assumption Risk Waiver
4. Cost for VBS is $20 per child or $40 per family. (Scholarships Available if Needed). 

Registration is complete when you drop off your payment in the Office.
